General Information

Office Hours/Appointments

8 am to 5 pm – Monday, Tuesday, and Thursday.
8 am to 4:30 pm- Wednesday & Friday.

*with exceptions to Holidays*

Patients are seen by appointment only.

Record Information and Changes

We request all patients, new and established, give accurate and complete demographic and insurance information. It is the responsibility of the patient to notify our office of any changes personal and/or insurance.

Missed Appointments/ Untimely Cancellation

Missed appointments represent a cost to us. If you are unable to keep your appointment, please give 24 hour notice. We reserve the right to charge for missed or untimely cancelled appointments. Excessive abuse of scheduled appointments may result in discharge from the practice.

Disability or Insurance Forms

There will be a $10 charge for the completion of medical forms. This includes all Short-term and Long-term disability forms, all FMLA (Family Medical Leave Act) Forms, and all forms for any Financial Institutions. Payment is due at the time forms are picked up. Forms are completed on Wednesday and Friday afternoons.

Treatment of Minors

A legal guardian must accompany a minor on their first visit and are responsible for payment. Written and/or verbal permission for treatment may be accepted.

Medications/ Refills

Prescriptions and refills will be addressed during office hours. Please try to call by Thursday if refills are needed for the weekend.

After Hours

If an emergency exist after hours or on a weekend, please go to Harton Hospital Emergency Room or call Harton Hospital at (931) 393-3000 and ask the operator to contact your doctor.

X-Ray Checkout Policy

Since Tullahoma Orthopaedics & Sports Medicine has moved to a new facility, we now have state of the art digital x-ray imaging. We no longer have x-ray developing equipment or copiers for old style analog x-ray images, which means we do not make regular x-rays. If you require a copy of an x-ray and it is in our digital processing system, we can print on paper, copy to a CD-ROM, or e-mail the image.

Our older x-ray files and current digital x-rays are part of your medical record. For copies of our x-ray film you may bring a camera and take a picture of the film.

We appreciate your understanding of this policy, and our desire to protect the medical/legal document that we have in our possession. Due to our busy schedule, your request may take 3-5 days to complete.
